Correcting Jaw Alignment

Ensuring Proper Tooth Contact
An open bite is when the upper and lower front teeth do not make contact when the mouth is closed. This leads to speech difficulties, chewing inefficiency, and often stems from thumb-sucking habits or tongue posture. Treatment aligns the teeth properly and restores full function.

Fixing Asymmetry for a Healthier Bite
A crossbite occurs when upper teeth sit inside the lower teeth instead of outside them. It can affect one tooth or a whole section of the bite, causing uneven jaw wear, gum recession on affected teeth, and facial asymmetry. Correcting it early prevents long-term damage.
Treatment options: Braces with bite correction techniques, palate expanders, or Invisalign with elastics.
Guiding Teeth into the Correct Position
When upper canines fail to erupt in the right position, they can grow at an angle into the arch, press against neighboring tooth roots, or become fully trapped in the gum tissue. Early detection with X-rays allows us to create space and guide the tooth down with braces or a minor surgical procedure. Left untreated, impacted canines can damage adjacent teeth and require extraction.
The AAO recommends an orthodontic screening by age 7. Catching an ectopic canine early makes treatment significantly simpler.
Treatment options: Braces with traction mechanics, clear aligners for mild cases, or surgical exposure combined with orthodontic eruption guidance.
A Conservative Solution for Missing Lateral Incisors
Some patients are born without their upper lateral incisors, the smaller teeth that sit between the two front teeth and the canines. This is called congenitally missing lateral incisors and it is more common than most people realize. Traditionally, the fix has been implants or bridges, which means waiting until growth is complete and committing to lifelong restorative work.
Canine substitution offers a different path. Instead of leaving space for an implant, we move the canines forward into the lateral incisor positions and reshape them with cosmetic bonding to look like lateral incisors. The first premolars then take on the role of the canines. The result is a complete smile made entirely of the patient’s natural teeth, with no implants, no bridges, and no future restorative cycles to plan around.
Treatment approach: Braces or clear aligners to move canines into the lateral incisor positions, followed by cosmetic reshaping and bonding by your general dentist. Case selection matters, so this approach is evaluated tooth by tooth based on canine shape, color, and bite relationship.
Bringing Teeth into Proper Alignment
Overjet refers to how far the upper front teeth protrude horizontally beyond the lower teeth. A large overjet can make it hard to close the lips fully, increases the risk of chipping a front tooth in an accident, and affects the overall balance of the face. Treatment repositions both arches to bring everything into proper alignment.
Treatment options: Braces with elastics, functional appliances, or clear aligners with precision attachments.
Enhancing Natural Jaw Development
When the upper and lower jaws grow at different rates or in the wrong direction, the result is a profile that looks imbalanced. Addressing this during the growth years with the right appliances can change the trajectory of jaw development, reducing the likelihood of needing jaw surgery as an adult.
Treatment options: Growth modification appliances, expanders, or braces in combination with orthopedic treatments during active growth.
Early Intervention for Long-Term Benefits
Phase I, or interceptive orthodontics, is limited treatment done around ages 7 to 10 when a specific problem will only get harder to fix if we wait. Common reasons include a narrow upper arch, a front-to-back jaw discrepancy, or severe crowding that is running out of space. Achieving Symmetry for a Balanced Smile
A shifted midline, where the center of the upper or lower teeth does not line up with the center of the face, often signals an underlying bite imbalance or asymmetric jaw growth. Midline correction is typically addressed as part of comprehensive treatment rather than as a standalone goal.
Treatment options: Braces with elastics, clear aligners with targeted adjustments, or bite correction appliances.
Complex Cases: Clear Aligners with Jaw Surgery
Correcting Skeletal Underbite and Open Bite
Some bite problems are skeletal in nature, meaning the jaw bones themselves are positioned incorrectly, not just the teeth. When this is the case, orthodontics alone can improve the teeth but cannot fully correct the bite or facial balance. A combined approach with an oral surgeon produces results that neither discipline can achieve on its own.
In the case below, a severe underbite and open bite were caused by a jaw position imbalance. Clear aligner treatment was used before surgery to align the teeth within each arch, and continued after surgery to fine-tune the bite. The result was a corrected bite, improved facial balance, and long-term functional stability.
Treatment approach: Clear aligners before and after orthognathic surgery, coordinated between Dr. Baharvand or Dr. Kang and an oral surgeon. The orthodontist and surgeon plan the case together from the start.
