What Is Airway Orthodontics?

Airway orthodontics looks at how a child breathes, sleeps, and grows. Traditional orthodontics focuses on straight teeth. Airway orthodontics considers how the upper and lower jaws, tongue posture, nasal breathing, and facial development may influence overall wellness.

Parents often come to us with questions about:

🔹 Mouth breathing

🔹 Snoring

🔹 Restless sleep

🔹 Bedwetting

🔹 Daytime sleepiness or difficulty concentrating

🔹 Grinding or clenching

🔹 Narrow arches or a crowded smile

🔹 Enlarged tonsils

🔹 Tongue posture and possible tongue tie

These are common concerns and do not mean something is wrong. They simply indicate the need for a thoughtful, evidence based evaluation.

What Is an Airway Orthodontist?

An airway orthodontist is an orthodontist who evaluates jaw development, breathing patterns, and sleep related symptoms as part of treatment planning. We do not take the place of pediatricians or sleep physicians. Instead, we partner with them to support your child’s overall health.

At Elate Orthodontics, we:

✔️ Screen for mouth breathing, snoring, and restless sleep

✔️ Evaluate maxillofacial and craniofacial development

✔️ Look at narrow palates, open bites, deep bites, and crossbites

✔️ Assess tongue posture and possible tongue tie

✔️ Consider whether the tongue has enough room based on jaw width

✔️ Identify forward head posture and its relation to breathing

✔️ Refer children to ENTs or sleep specialists when necessary

You will always receive personalized guidance. No two children grow the same way and no two treatment plans should look the same.

How We Integrate Airway Considerations Into Treatment Planning

Step 1

Airway Screening

We begin by asking questions about sleep, breathing habits, and daily behavior. During the visit, we also observe breathing patterns while your child is relaxed.

Step 2

Growth and Development Evaluation

We evaluate the width of the upper jaw, the position of the lower jaw, the bite relationship, and how the face is growing. We also examine the space available for the tongue.

Step 3

Collaboration With Medical Providers

If a child shows signs of sleep disordered breathing, enlarged tonsils, or airway obstruction, we refer to pediatricians, ENTs, or sleep physicians. These specialists may order sleep studies or further medical evaluation.

Step 4

Orthodontic Treatment That Supports Healthy Breathing

Some children benefit from palate expansion when the upper jaw is too narrow. Others may need guidance with tongue posture or myofunctional therapy. Teens and adults may benefit from clear aligners or braces that create better bite relationships and more room for the tongue. Learn more on our clear aligners page.

Step 5

Evidence Based Recommendations

Our responsibility is to treat your child appropriately, not aggressively. We do not recommend expanders for every child because the research does not support universal treatment. When expansion is needed, we explain why, how it works, and what improvements we expect to see.

Signs Your Child May Benefit From an Airway Evaluation

Many parents in Frisco, The Colony, and Prosper come to us after noticing one or more of the following signs in their child. None of these on their own are a cause for alarm, but together they can paint a picture worth evaluating.

During sleep:

🔹 Sleeping with the mouth open consistently

🔹 Snoring or noisy breathing during sleep

🔹 Tossing and turning or waking frequently

🔹 Bedwetting beyond typical age

🔹 Grinding or clenching teeth at night

During the day:

🔹 Chronic mouth breathing even when not sick

🔹 Daytime sleepiness, low energy, or difficulty concentrating in school

🔹 Behavioral issues sometimes mistaken for ADHD

🔹 Frequent headaches or dark circles under the eyes

🔹 Tongue resting between or against the front teeth

In the mouth and jaw:

🔹 Narrow upper arch or visible crowding of the front teeth

🔹 Crossbite, open bite, or deep bite

🔹 Enlarged tonsils or adenoids noted by a pediatrician

🔹 A short or restricted frenum (tongue tie)

If you are seeing several of these signs, an airway evaluation at Elate Orthodontics is a good next step. It is not a commitment to treatment. It is simply a conversation. Schedule a free consultation here.

Airway Evaluation for Children

Airway related symptoms often appear during early childhood. If you notice snoring, sleeping with the mouth open, restless sleep, behavioral concerns, or difficulty focusing, an orthodontic evaluation may help clarify what is happening.

The American Association of Orthodontists recommends that children have their first orthodontic evaluation by age seven. At this age, we have enough information to identify developing jaw and airway concerns while there is still ample time to guide healthy growth. Early intervention is not always necessary, but early evaluation is always worthwhile.

During the exam, we assess maxillary width, tongue posture, bite patterns, facial growth direction, signs of mouth breathing, and possible functional habits. If needed, we collaborate with ENTs or myofunctional therapists who specialize in breathing and tongue function.

Our goal is to identify issues early and guide healthy growth while avoiding unnecessary treatment. Learn more about our approach to pediatric orthodontics.

Airway Focused Orthodontics for Teens and Adults

Teens and adults may also struggle with airway related concerns. These often include grinding, snoring, daytime fatigue, poor sleep quality, or TMJ discomfort. In these cases, orthodontic treatment can help improve jaw alignment, tongue space, and breathing efficiency.

Airway focused treatment for older patients may include braces or clear aligners to correct bite issues, expansion when appropriate, oral appliance therapy through a sleep physician, and collaboration with medical professionals for sleep apnea management.

Orthodontic Treatments That Support Airway Health

Treatment 1

Palatal Expansion

Palatal expansion widens the upper jaw and can improve nasal airflow by widening the nasal floor, which shares a wall with the palate. It is helpful for some children but not needed for every child. We recommend it based on anatomy, symptoms, and age, and explain the expected outcomes clearly before proceeding.

Treatment 2

Clear Aligners for Airway Support

Clear aligners are effective for creating better bite relationships and can help support healthier tongue posture by creating more space in the arch. They are especially useful for teens and adults managing airway related concerns alongside cosmetic alignment goals.

Treatment 3

Myofunctional Therapy

Myofunctional therapy strengthens the tongue and lip muscles, improves nasal breathing habits, and corrects resting tongue posture. It works well alongside orthodontic treatment and is often recommended before or after expansion to reinforce healthy function. We refer to qualified myofunctional therapists in the Frisco and Colony area.

Treatment 4

Oral Appliance Therapy

For adults diagnosed with sleep apnea or upper airway resistance syndrome, sleep physicians may prescribe an oral appliance that repositions the lower jaw to keep the airway open during sleep. We coordinate care closely for these patients to ensure the appliance works alongside any orthodontic goals.

A Note on Airway Information Online

Parents today are exposed to a lot of airway content on social media, and some of it is alarming, oversimplified, or commercially motivated. We want to be direct with you about this.

The relationship between orthodontics and airway health is real and supported by clinical research. Palate expansion, jaw development, and tongue posture do influence breathing. However, not every child who snores needs an expander. Not every narrow arch requires aggressive early treatment. And an orthodontist is not the right provider to diagnose or treat sleep apnea on their own.

At Elate Orthodontics, we follow the evidence. We do not use fear to drive treatment decisions. If your child needs intervention, we will tell you clearly why, what we expect it to accomplish, and what the alternatives are. If watchful waiting is the right call, we will tell you that too.

If you have seen something online that concerned you or raised questions about your child’s breathing or jaw development, we welcome that conversation. Bring your questions and we will give you honest answers.
